Play the game as normal, as the expansion pack is only an option if you want more active quests (the shivering isles).
Better still, get the Game of the Year Edition, as it already has the shivering isles expansion pack included in it.

Game of the year has to expansion packs already to go when you start playing. They are included in the main story and on the maps. So if you head to the shivering isle's you doing a an expansion. The other expansion is knights of the nine, which has you roaming over the map questing to become a knight.
