She kinda looks like an old "Sniper Wolf" especially the way her blouse is undone.

BUT Sniper Wolf was killed at Shadow Moses which took play 9 years before MGS4. If it was Wolf than she has aged rapidly (and no she wouldnt age that badly because of FOXDIE or anything cause she was immune to FOXDIE).

Its definately not "The Boss" from MGS3 because she would be about 50 or 60 by now... and Naked Snake killed her, thus assuming the title of "Big Boss."

Beyond that it could be a relative of the current characters or a new character altogether.
