According to the website it will cost you $1 to load up the card each time and 0.25c every transaction.. but if your under 18 that the price you have to pay.

cost me under $20 for the custom card. cant remember exact price as it was a year ago.
Fee i have noticed:
$1 load fee - i load using bday. i think i had to pay $8 for the 100 id check which i did at the local newsagent. but i think you can transfer via netbank and stuff now too.
25cent for domestic buying and 50 cent for international
and i just past a year and they sent me out a new card and i didnt get billed or anything. so im guessing no yearly fee
